Autopsy, examining the body after death, offers peace of mind to family members by pinpointing the cause of their loved one's illness. It also aids researchers in their quest to cure Alzheimer's and related memory disorders. It promises better health for future generations of your family and countless other families affected by these devastating diseases.

The Gift to Life Outpatient Autopsy Planning Program is designed to educate patients and their families about the many benefits of postmortem examinations and to help make advance arrangements for autopsies. It facilitates access to services but does not pay for or directly provide them.

How do I get more information?

• If you have questions, please contact the Gift to Life Program at (801) 585-9989. We are available to assist you during regular hours.
• For general information about the importance of autopsies and postmortem diagnosis, you may also contact the Utah Chapter of the Alzheimer's Association by calling (800) 371-6694, or visiting them online at www.alz.org/utah.

How do I enroll for the programs?

• Planning should begin as soon as you decide
• Outpatient autopsy preparations must be made before death; we cannot assist families unless they have enrolled in advance
• To indicate your intent to obtain an autopsy, please begin by completing the enrollment form and faxing it to (801) 581-2483
