For plain film, exams that are not scheduled in advance, print green radiology requisition form and use the stamper plate to emboss the greencard directly on the form. Also write study visit # on requisition.

The Patient Care Classification form has been updated and renamed. The Protocol Billing Grid should be completed for each new clinical research project. Determiniation of Research and Usual Patient Care should be clearly documented in the grid. This document should be used as a guide in creating the Research Budget and Informed Consent Form.
