Billing Information

In order to serve you better, we would like to provide some information regarding out billing:

• An examination and evaluation by the Dentist
• Any dental procedures done on the date of service

For our patients with dental insurance, a $15 co-pay will be expected at each visit.

For our patients who do not have dental insurance, we offer a 10% discount in the full amount of the bill is paid on the date of service. If the full amount cannot be paid at each visit and the treatment estimate is more than $200, payment arrangements must be made with a Hospital Financial Counselor before treatment can continue.

We offer a 20% discount to University of Utah students who do not have dental insurance if the full amount is paid on the date of service. A current student ID is required.

As a courtesy to you, the University Hospital will bill your insurance company. However, you will be held personally responsible for charges not covered by your insurance company. Therefore, it is in your best interest to find out what your insurance company does and does not cover.
