Dental Residency Program (GPR): Application Process

Application materials must be completed to PASS by December 18, 2011. Our program deadline is January 1, 2012, with interviews also to be completed by January 1, 2012.

Application

Applications are made through the Postdoctoral Application Support Service (PASS). There is no separate application required; however, we would like to receive a small recent picture from you.

We participate in MATCH and our MATCH program code number is:
9335
Applicants will be notified by Match in January 2012.

Interviews

Personal interviews are required prior to January 1, 2012.
Residency applicants should call (801-581-8951) and schedule an interview/visit to our facilities prior to our final closing date of January 1, 2012. These visits permit the applicant to gain a firsthand view of the hospital, the dental services and resources, and at the same time provide the staff with an opportunity to assess individual attributes and aspirations of the applicant.

We welcome your application to our program. Please apply through PASS, complete your application to PASS prior to December 18, 2011, and send or bring a recent picture to us directly at the following address by January 1, 2012:
