Selection Criteria
- Students pay for schooling up front and are eligable to receive their tuition reimbursement money when they return to the
State of Utah to practice dentistry.
- Return to full time dental practice in Utah within one year of graduation from dental school, dental graduate program or military scholarship obligation.
- $20,000/year for a maximum of 4 years.
- $26,666/year for a maximum of 3 years (rural or underserved area).
If recipient moves out of rural practice, then payment reduces accordingly.
- Recipients should request funds in writing. Submit request between January 1 and April 15 of first practice year and then
each subsequent year. Funds paid between April 15 and June 30.
- Deferment for dental graduate programs or military scholarship obligation.
- Money for Program
- Money will be set aside before student enters school.
- Money will be held at the University of Utah in a special restricted account.
- Program Dates
- Application deadline is April 15, 2012.
- Award Notification June 1, 2012.
- To qualify an applicant must be a Utah resident for 5 years or more at the time of application to dental school
(September 1, 2011).
- Yearly Status Reports Required
- Yearly transcript from applicant's school.
- Verify status of state residency.
- If awarded major scholarship, financial award/assistance, tuition reduction, etc., for dental school
(greater than $7,000 per year).
- If receive a scholarship for difference between in-state and out-of-state tuition.
- Change of tuition status from out-of-state to in-state student, then the student would not be eligible
for the remaining years. He/She would be eligible for repayment assistance for year(s) prior to change
in status.
- Military scholarship - not eligible for remaining years after receiving military scholarship.
- If holding position in the RDEP program - not eligible for "At Large" position.
- Foreign dental or non-ADA accredited dental school students are not eligible for "At Large" positions.
- Students shall meet the following minimum requirements:
- Accepted to a US accredited dental school.
- Scholastic Cumulative Overall minimum GPA of 3.3 as recorded on AADSAS application.
- DAT scores - 17 ACA, 17 PAT minimums.
- Financial need.
- Experience or observation in a dental office.
- Community service or leadership experience.
- Underserved consideration - from rural or underserved area as defined by Utah State Health
Department. All counties in the State, except Utah, Salt Lake, Davis, Washington and Weber are designated
rural counties.
- Personal statement essay including why this funding will benefit you (limit 1 page).
- Materials to submit:
- Application form.
- Copy of your Letter of Acceptance to a US accredited dental school.
- Copy of ADSSAS application as found online.
- Copy of DAT Scores.
- Proof of Financial Need (Copy of FAFSA application, SAR, or copy of school financial award letter).
- Personal statement essay including information about you and your family and why this funding will benefit you (limit 1 page).
