*Terminating Physicians - Your incentive balance will be paid out in your July 07, 2009 paycheck.
NOTE: Once you are terminated from the payroll system, we will no longer be able to pay out your incentive credits. So if you terminate earlier than June, you need to request your Incentive to be paid out in one of the previous scheduled pay out dates. We do not receive any information on terminating physicians until May.
