By logging on, you agree to the following University of Utah Health Sciences Center Policies:

• You may ONLY access patient information in PACSWeb Viewer if (a) you are responsible for providing care to the patient, or (b) your job requires you to access the patient's chart.
• You MUST log out after using PACSWeb Viewer. The University tracks and audits information accessed by your account name and password.

Failing to comply with these guidelines, and failing to protect patient information generally, are grounds for disciplinary action, up to and including TERMINATION of your employment, and may subject you to CIVIL DAMAGES and CRIMINAL PROSECUTION.
