Welcome to the Account Management Website where you will find information and tools for supervisors to request and terminate access to the network and clinical applications.

New Account Request Application & Process:
The Account Request Application has a new design and added functionality that make it easier for you to request computer accounts.

The Account Termination Application has been added to make it easier for you to terminate computer accounts. Please do not contact the Help Desk nor submit a Trouble Ticket to request termination of any accounts.

Please do not submit a Trouble Ticket for non-employee account requests.  Follow the instructions below outlined for individuals without a PeopleSoft ID, beginning with Step 5.

With the new Account Request Application comes a change to how Information Security & Confidentiality Agreement forms are processed. Completed forms should NOT be faxed to ITS, nor Human Resources.  Signed forms MUST be kept on file within the department. Employees will now be added to the HSC system through the daily Human Resources database feed, so there is no need to send the forms to ITS.



Obtain a new account(s) for staff, faculty, students, volunteers, or others who have a PeopleSoft ID.

First, you must have applied to be an account requestor.

If you are a registered account requestor, follow these steps:

  1. Has the individual for whom you are requesting an account signed the "Confidentiality and Information Security Agreement"?
    • If yes, skip to step 4

  2. Have the individual sign the "Confidentiality and Information Security Agreement".


  3. File the completed form in the employee's personnel file within the department.  Please do NOT fax the form to the ITS Help Desk nor to Human Resources.


  4. Login to the "Request Account" and follow the instructions provided.

First, you must have applied to be an account requestor.

If you are a registered account requestor, follow these steps:

  1. Fill out the Non-Employee PeopleSoft request form and follow the instructions included.  Once you receive the PeopleSoft ID, proceed to the next step.


  2. Has the individual for whom you are requesting an account signed the "Confidentiality and Information Security Agreement"?
    • If yes, skip to step 5

  3. Have the individual sign the "Confidentiality and Information Security Agreement".


  4. File the completed form in the employee's personnel file within the department.  Please do NOT fax the form to the ITS Help Desk nor to Human Resources.


  5. Login to the "Request Account" application.


  6. Enter the non-employee ID in the appropriate field.


  7. If the name does not appear in the results, click on the "Can't find the person you are looking for?" link.


  8. Then click on "Select to Request Non-employee Access."


  9. Enter the non-employee ID information and submit the request.
