Note: You must be an account requestor to request accounts. You can apply to be an account requestor here.

How to request a new account for users who have a uNID:

 

If you are a registered account requestor, follow these steps:

  1. Confirm the user has signed the Information Security & Confidentiality Form
  2. Login to the Account Request Application and follow the instructions provided.

How to request an account for a user who does NOT have a uNID:

 

If you are a registered account requestor, follow these steps:

  1. Complete the University Affiliate/Non-Employee Form and follow the instructions included.  Once you receive the uNID, proceed to the next step.
    1. To be processed properly, you must select 10035 - Incoming New Hire.
  2. Confirm the user has signed the Information Security & Confidentiality Form
    • Note: File the completed form in the employee's personnel file within the department.  Please do NOT fax the form to the ITS Help Desk nor to Human Resources.
  3. Login to the Account Request Application and follow the instructions provided.
  4. Enter the uNID in the appropriate field.
  5. If the name does not appear in the results, click on the "Can't find the person you are looking for?" link.
  6. Then click on "Select to Request Non-employee Access."
  7. Enter the uNID information and submit the request.
