Established to Serve Utah's Disabled MinersThe Miners Clinic is held on Fridays at the University of Utah Parkway Community Cinic. The Univeristy of Utah Parkway Community Clinic is located at 145 West University Parkway, Orem, Utah. See Parkway Clinic Directions for map and directions. 
Patients are seen initially by a physician under the direction of Joseph Webster, MD, Miners Hospital Medical Director.
To request appointment in the Miners Clinic, please call 1-801-581-2251 or the toll free number 1-866-864-6377. You will be asked to complete and return the Miners Hospital Application for Services before your initial appointment is made. The application is located under Information. You can request that the application be sent to you. |
