Frequently Asked Questions

Q: Why does the credentialing process take so long?

A: Obtaining a Utah license and a faculty appointment can take several months. These must be in place before an application can be presented for approval.

Because the bylaws and JCAHO require primary source verification, we are often dependent upon letters we send out regarding previous affiliations, peer reviews, academic training, and any malpractice information, etc. It is imperative that your application be filled out in detail with regard to addresses and contact information.

Q: Is the option to be granted temporary privileges offered?

A: Temporary privileges can only be granted for the care of a specific patient in an emergency situation, or for the purpose of proctoring or teaching by visiting professors.

Q: Can I be granted privileges in more than one department?

A: Yes. Often times physicians are boarded in multiple specilaties. In this case, both privilege delineation forms will need to be filled out and necessary approval signatures obtained from both departments. Faculty appointments would also be required in both departments.
