Drug Defect Reporting
Ever had a drug product you couldn't use? If so, please let the Drug Information Service (DIS) know. We want to report defects. This helps the manufacturer with their quality control and also gives us credit for product we cannot use. The DIS will call the manufacturer to notify them of the problem. You may send defects to the DIS (421 Wakara Way, Suite 204) or leave the defect with Melissa or Kathy at the front desk of the pharmacy (UHOSP A050) where the DIS staff will pick it up. Please be sure to include a detailed description of the defect and, more importantly, your name and contact information in case we need to clarify.
